He has shoulder length pale yellow hair that covers his right eye. His left eye is visible and is blue.
Plot[]
He first appeared in episode 41 with the rest of the team playing against Raimon in the finals. At the end of the episode, the score was tied due Kurosaki scored a goal with Ballista Shot and Tsurugi with Death Drop, which Shoot Break couldn't stop.
In episode 42, he passed the ball to Kurosaki, who released his Keshin Enma Gazard. At the end of the first half, the score was tied again due Kurosaki released his Keshin and scored with Bakunetsu Storm and Nishiki released his Keshin too and scored with Bushin Renzan. He didn't appeared in the second half because Seidouzan was replaced by Dragonlink.

In order to recruit Koizaki, the following requirements are needed:
- Player: Okegawa Yukimori
- Item: Passionate Coach's Teaching (熱血コーチの教え, randomly dropped from Seidouzan in Furukabu's lower taisen route)
- Photo: Ashored Coconut (流れ着いたヤシの実, taken at the Water Path to the Water World Stadium)
- Item: Daichi no Glove (だいちのグローブ, can be randomly dropped from Chuugaku Senbatsu α or Chuugaku Senbatsu β in Geimu Konomu's upper taisen route)
After this, he can be recruited for an amount of 2800 Yuujou points.

- His name, Koizaki Io, is derived from コイオス (read as "Koiosu"), which, in turn, is the Japanese pronunciation of Coeus, the Greek Titan with no particular domain who was a member of the Titans, a reference to Seidouzan's naming motif.
- His dub forename, Coeus, is a reference to the eponymous Greek Titan, which is also a reference to Seidouzan's naming motif.
Interestingly, his dub surname, Heavens, is the literal meaning of Coeus' other name, Polus.
